Fuwai Hospital Expert Team Performs Indonesia's First LVAD Implantation at RSCM

Clinical Pain Points & Technology Introduction Recently, the Left Ventricular Assist Device (LVAD) team from Fuwai Hospital, Chinese Academy of Medical Sciences, traveled to Dr. Cipto Mangunkusumo National Central General Hospital (RSCM) in Indonesia to assist in completing the country's first LVAD implantation.

Technical Background: For a long time, the treatment of end-stage heart failure in Indonesia has been severely constrained by the scarcity of heart transplant donors. To break through this clinical bottleneck, the Fuwai team introduced the third-generation fully magnetically levitated artificial heart (Corheart 6), successfully establishing a stable mechanical circulatory support (MCS) system for a critically ill patient with an LVEF of 14%.

Standardized Surgical Pathway & Hands-on Proctoring This surgery was not merely a device implantation, but a transnational translation of standardized surgical pathways. During the procedure, the Fuwai team provided full-process, hands-on proctoring to the Indonesian cardiac surgery team led by Dr. Arza Putra. The core steps were as follows:

  1. CPB Establishment: Under general anesthesia, a median sternotomy was performed, and cardiopulmonary bypass (CPB) was established to ensure hemodynamic stability during intracardiac maneuvers.
  2. Apical Coring & Pump Fixation: Precise positioning and coring at the left ventricular apex were performed to implant the pump. The inflow cannula angle was strictly calibrated to avoid ventricular septal suction events, ensuring the flow field distribution met fluid dynamics requirements.
  3. Outflow Tract Anastomosis: An end-to-side anastomosis was performed between the artificial graft (outflow tract) and the ascending aorta, completing the construction of the mechanical circulatory pathway.
  4. Ultrasound-Guided Speed Titration & Weaning: Following meticulous de-airing, pump speed was finely titrated under real-time transesophageal echocardiography (TEE) monitoring to balance biventricular function, allowing for smooth weaning from CPB.
